Dr. Jerby/Susan Jerby NP will be out of town July 15th-July 30th. They will be checking the portal messages periodically, and will answer messages that cannot wait until they return. For urgent/emergent matters, please go to urgent care or call 911. 

The office will be open 9:00-4:30 during this time for scheduling and supplement purchases.

PLEASE NOTE: Follow up appointments with Dr. Brian Jerby are $249 for up to 30 minutes and with Susan Jerby NP are $199 for up to 30 minutes.  Any visits LONGER THAN 30 minutes will be prorated at this rate. We do not issue superbills at this time for any visits.

Dear Friends--We are committed to providing you expert, timely care. We have hundreds of messages to answer every day. Please keep your portal messages as short and succinct as possible, numbering multiple questions . Please use the portal for appointment requests, prescription refills, treatment clarification and updates between appointments. If your question requires medical expertise involving a medication/treatment change, new symptoms, evaluating tests or labs ordered by an outside provider, or more than a brief response, you will be asked to make an appointment. Please do not send multiple messages as this slows down our ability to respond to everyone and when you send a new message, it bumps all your messages to the end of the queue and will then take us longer to get to your messages. **Please note that patient portal messages and phone calls will be answered Monday through Thursday. We are out of the office most Fridays. If you have an urgent need, please call the office number at 980-339-5155** If you have a medical emergency, please call 911 or go to your nearest emergency room.
